WHAT IS A POST–WEIGHT LOSS TUMMY TUCK?
Significant weight loss is an incredible accomplishment. Whether the weight loss occurred through diet and exercise, medications such as GLP-1 agonists, or bariatric surgery, many patients ultimately find that the skin of the abdomen does not “bounce back” the way they had hoped. A tummy tuck, or abdominoplasty, is one of the most common procedures performed after weight loss to remove excess abdominal skin, reshape the waist, and restore a smoother abdominal contour. During weight gain, the skin stretches to accommodate the increased volume of the abdomen. When the weight is lost, the skin often lacks the elasticity required to retract fully.
This is particularly true after massive weight loss, where the skin has been stretched for a prolonged period of time. The result is loose, draping skin around the abdomen that can create hygiene concerns, rashes within the folds, and difficulty finding clothing that fits well.

One of the most important considerations for weight loss patients is weight stability. I generally recommend that patients maintain a stable weight for at least six months before surgery. Many patients notice that skin laxity actually becomes more apparent several months after the weight loss occurs. This happens because the skin continues to relax once the underlying volume is gone. Waiting until this process stabilizes allows us to plan surgery more accurately and achieve a better long-term result. Patients who continue to lose weight after surgery may develop new skin laxity, which can compromise the outcome.
A CLOSER LOOK AT BODY CONTOURING AFTER WEIGHT LOSS

TYPES OF POST–WEIGHT LOSS ABDOMINAL PROCEDURES

After significant weight loss, a full tummy tuck is far more common than a limited or mini procedure. This surgery removes the loose skin from the lower abdomen, tightens the abdominal wall, and repositions the belly button as the skin is redraped. For many weight loss patients, the amount of skin removed can be substantial. The incision is placed low on the abdomen so it can be hidden beneath underwear or a swimsuit.
LIPOABDOMINOPLASTY
Many patients still have areas of residual fat that can be sculpted at the time of surgery. Liposuction is often performed along the waistline, flanks, and mons region to improve the overall contour of the abdomen. When liposuction is combined with skin removal, the procedure is referred to as a lipoabdominoplasty.
CIRCUMFERENTIAL BODY LIFT
For patients with more significant skin laxity around the entire torso, a circumferential body lift may be recommended. A body lift extends all the way around the trunk, removing excess skin from the abdomen, flanks, and lower back in a single procedure. In addition to flattening the abdomen, this procedure can also lift the outer thighs and buttocks, restoring a smoother, more balanced silhouette. This is one of the most powerful procedures available for patients who have undergone massive weight loss.
THE PROCEDURE
A tummy tuck or body contouring procedure is performed as an outpatient surgery in our AAAASF-accredited Ambulatory Surgery Center located in the Nordstrom Medical Tower in downtown Seattle.
Surgery is performed under general anesthesia and typically takes between 2–4 hours, depending on the complexity of the procedure and whether additional areas such as the flanks or back are treated.
The procedure begins with careful surgical markings on the morning of surgery to identify the areas of skin removal and any planned liposuction. Liposuction is often performed first to sculpt the waist and improve the overall contour of the trunk.
An incision is then made low on the abdomen. The abdominal skin is elevated so that it can be redraped smoothly. If the abdominal muscles have separated, they are repaired in the midline to restore the strength and contour of the core. Once the excess skin is removed, the remaining skin is carefully tightened and closed using multiple layers of absorbable sutures. Drains are commonly placed and are typically removed once drainage decreases to an appropriate level. The final step of surgery is the creation of the new belly button opening, followed by placement into a compression garment before you return home to recover.
RECOVERY AFTER SURGERY
Recovery after a tummy tuck or post-weight loss body contouring procedure requires patience, but most patients are pleasantly surprised by how quickly they begin to feel better. Patients are encouraged to walk the evening of surgery to promote circulation and reduce the risk of blood clots.
Common aspects of recovery include:

WHY SKIN DOESN’T BOUNCE BACK AFTER WEIGHT LOSS
One of the most common questions I hear from patients after significant weight loss is: “Why didn’t my skin tighten back up?” The answer comes down to skin elasticity. Our skin contains structural proteins—primarily collagen and elastin—that allow it to stretch and then recoil. During periods of weight gain, the skin stretches to accommodate increased volume in the abdomen and trunk. If this stretching occurs over a long period of time, those elastic fibers become permanently weakened. When the weight is eventually lost, the underlying volume disappears, but the skin often cannot fully retract.
This effect becomes more pronounced in several situations:

Many patients also notice that loose skin becomes more apparent several months after the weight loss occurs. Unfortunately, once the elastic fibers of the skin have been stretched beyond their capacity, exercise cannot tighten the skin itself. While strengthening the abdominal muscles can improve core stability and posture, it does not remove loose skin. This is why body contouring procedures such as a tummy tuck or body lift are often the only way to remove the excess skin that remains after significant weight loss.

What is the difference between a Tummy Tuck and Liposuction?
Liposuction only removes fat; it doesn't address loose skin or separated abdominal muscles. A Tummy Tuck does it all—it removes excess skin, eliminates stubborn fat, and repairs the "diastasis recti" (muscle separation) that often occurs after pregnancy or significant weight loss. Most of my patients benefit from a combination of both.

Will a Tummy Tuck remove my stretch marks?
A Tummy Tuck will remove the stretch marks located on the skin that is physically excised—typically the area below your belly button. Any stretch marks above the belly button will remain, but they are often shifted lower and tightened, making them much less noticeable.

How long do I need to wait after weight loss surgery to get a Tummy Tuck?
I generally recommend waiting 12 to 18 months after your weight loss surgery (like a gastric sleeve or bypass). This ensures that your weight has stabilized and that your body has recovered from the nutritional changes associated with rapid weight loss. Being at your "goal weight" for at least six months is the gold standard for safety and results.

Can I have a Tummy Tuck if I plan on having more children?
While it is physically possible, I strongly recommend waiting until you are finished having children. Pregnancy can stretch the skin and separate the muscles again, which may undo the beautiful results of your surgery. Waiting ensures that your investment in yourself lasts a lifetime.
